You must complete (3) Live Scan forms. This form can also be found on the California Department of Public Health website. Follow the livescan sample when completing your form.

*This form must be completed prior to the first day of instruction.

Scrubs

We have partnered with a new scrubs vendor ScrubLyfe Uniforms.

Please keep in mind that we are highly recommending that two sets of scrubs be purchased.

Uniform attire is required by 1st day of clinical. CNA program requires a watch with 2nd hand and black or royal blue stethoscope.

Please be advised that no absences or tardies are allowed. 100% attendance is required by the state for certification.
